What is peptide therapy? An honest look at benefits, evidence, and safety

By Alison Malizzi, MSN, AGACNP-BC · October 15, 2026

Peptide therapy is one of the most talked-about areas of longevity medicine, and one of the most confusing. Here's a plain-language guide to what peptides are, what the evidence shows, and how to approach them safely.

What are peptides?

Peptides are short chains of amino acids, the same building blocks that make up proteins. Your body makes thousands of them, and they act as signals that help regulate hormones, appetite, metabolism, healing, and more. Peptide therapies use natural or lab-designed peptides to influence those signals.

Not all peptides are the same

This is the most important thing to understand. Peptides used in medicine fall into very different categories:

  • FDA-approved peptide medications. Some of the most widely used drugs today are peptides, including insulin and GLP-1 medications used for diabetes and weight management. These have been through large clinical trials.
  • Compounded peptides. Some peptides can be legally prepared by licensed compounding pharmacies for individual patients. They are not FDA-approved, and the FDA's rules on which peptides may be compounded have changed in recent years and continue to change.
  • "Research-use" peptides sold online. Products labeled "for research use only" aren't labeled for human use. If you're considering one, talk with your provider first so you can review it together.

What can peptides help with?

Depending on the specific peptide, people use them for metabolic health and weight management, recovery, sleep, skin health, and body composition, including increasing lean muscle mass and decreasing body fat. The quality of evidence varies a lot. Some peptides have strong human data, while many popular ones have mostly animal studies or small trials. A good clinician should tell you honestly which is which.

Questions to ask before starting

  1. Is this peptide FDA-approved, or compounded?
  2. What human evidence supports it for my goal?
  3. Which licensed pharmacy will prepare it?
  4. What side effects should I watch for, and how will we monitor them?
  5. How will we know if it's working, and when would we stop?

Peptides are one piece of the picture

Peptides work best as part of a broader plan that also looks at hormones, sleep, nutrition, strength training, and metabolic labs. At Vida Avanti, a peptide consultation starts there. We only prescribe peptides that can be legally dispensed, and we explain the evidence, unknowns, and costs before you decide.
